Naturopathic New Patient Intake Forms

Please read the Welcome Letter before filling out the forms

Once you are ready to schedule just call the office during regular office hours to schedule with our receptionist. For your first visit at The Center for Integrative Wellness you need to read the New Patient Welcome and HIPPA Notice of Privacy Practices. For your first appointment you will need to complete the New Patient Questionnaire and sign the Declaration and Consent for Naturopathic Care.
